This feedback form is meant for all levels of instructors, from candidates, all the way to lead instructors or instructor mentors with many courses under their belt. Below are general guidelines and expectations for submitting peer feedback to Base Medical.
We encourage instructors to share kind and assertive feedback with one another in-person, especially if working for the same organization or education center, as this fosters a transparent and collaborative culture amongst the Base Medical team.
We require names to be shared for tracking purposes, but if you wish for your feedback to be anonymous, please mark this on the form and someone from Base Medical will follow-up with you.
Please use this space for both positive and constructive feedback. While constructive feedback can often be helpful, we also want to celebrate success and uplift exceptional instructors
Feedback that is growth-oriented, specific, clear, and respectful is the most helpful. Feedback that is disrespectful or disparaging is not only not productive, but is not welcome.
