To ensure that Base Medical is maintaining high standards and that our instructors are remaining up to date on best practices and curriculum updates, all Base Medical Instructors must renew their certification every two years.

Use the below form to provide us with your most up-to-date information. By completing this form, you’re also confirming that you are planning to teach the required minimum of 2 courses per year during this certification cycle. This is also what you will use to provide updated credentials.

Please note that starting with the next certification cycle (2028), instructors will be charged a $35 fee for their recertification.
