Candidate Feedback Instructions
Participating in a mentorship course and receiving feedback is a required step for candidates to complete before being certified as lead instructors. Goals for mentorship course are for candidates to lead one skills lab and one scenario during the practical training. The candidate evaluation form also outlines general expectations and assessment criteria for the candidate.
There is a paper copy of this candidate evaluation form (linked below) and it is the candidates responsibility to share this form with their instructor mentor prior to a mentorship course. Instructor mentors are advised to review and use the paper copy of the candidate evaluation as needed. However, submitting an electronic version here, via the the website form below, is the best way for Base Medical to collect candidate evaluations.
